The last patient (80) of the clinical trial, approved by Clinical Trials of EUA, was treated today.

The Clinical Trial started on September 1, 2014and finished the recruitment today, March 8, 2018 was very difficult.

Despite our efforts contacting the Association and Urology College, the Association of family physicians and personally several doctors, we dis not received any patient from these colleagues, although the study had no cost to patients.

This is the big study that all doctors are waiting, because it will demonstrate that patients improvement because it, is effective and not by the placebo effect. Patients were selected according to criteria imposed by the Clinical Trials.

On the day of the exam they opened an envelope, whose content they only had knowledge after 6 months.

In the meantime they made tests at 1, 3 and 6 months. Half of the patients was right then submitted to the treatment and the rest only after 6 months, all of them was placed a catheter, however, those of the placebo group nothing was done to them, and therefore they have not improved.

The Clinical Trial only will end in one year, because patients have to do tests periodically for 1 year

With the publication of the results of the Study, whose recruitment and treatment ends today, there will be no doubt that the embolization of the prostatic arteries, for the treatment of benign prostatic hyperplasia, of which we are the world pioneers, is effectively sucessfull and with minimum risks, under local anesthesia and ambulatory.

“The 6-month- 7.5 year Outcome of Prostatic Artery embolisation in 1100 patients with benign Hyperplasia”

PurposeTo evaluate the short, mid and long term outcome of prostatic artery embolization (PAE) for patients with benign prostatic hyperplasia (BPH).

Materials – Between March 2009 and September 2016,   1100 patients with BPH underwent PAE. One hundred eighteen patients were in acute urinary retention (AUR) and 221 patients had a prostate volume over 100cm3. All patients were evaluated up to 5 months, 982 patients at mid term between 1 and 3 years and 600 patients at long term between 3  and 7.5 years.

Results – Mean patient age was 68.7 years. There were 18 (1.6%) technical failures. The cumulative clinical success rates at short, mid and long term follow up were 90%, 84.1% and 79.2% respectively. The results of the 118 patients in AUR, were: 110 (93.2%) had the catheter removed between 2 days and 3 months; 106 (89.8%) and 99 (83.9%) have good results at mid and long term, respectively. The results of the 220 patients with prostate volume larger than 100cc were: 190 (86.4%) with short term clinical success, 178 (80.9%) and 171 (77.7%), respectively, at mid and long term clinical success. There were 2 major complications, a bladder wall ischemia and a perineal pain for 3 months,

Conclusions – PAE is a safe procedure for patients with BPH with good short, mid and long term results.

  • Martins Pisco received an ward by the paper “Medium and Long Term Results of PAE in 630 Cases”, published in JVIR in 2016 and was considered one of the best publications. It is the 2nd time that he. received such a distinction.
  • The presented paper: “Short, Medium and Long Term outcome of prostate artery embolization for patients with benign prostatic artery embolization: 1000 patients” was the best amongst 1354 and for this reason was considered the “Abstract of the year and numbered as Nº 1 and Classified as distinguished Abstract”

Sorry for the slight delay in writing to you after my training with you in prostatic embolization. I was so busy when I came back due to the fact that it was opening of classes in medical school. I had to prepare a lot of things especially the syllabus for the whole school year 2015-2016. i just came back from a strategic planning workshop for the faculty. Anyway I hope you are in the best of health.

I had a chance to see in the internet our picture with Richard. i showed it to the other members of the medical staff and they were happy to note that I was well trained with 14 patients in two weeks. they were also impressed that your patients come from other countries outside of Portugal and even outside of Europe. They were also amazed that you could finish 7 patients in one day.

By the way, during my busy schedule when I came back, I already have two invitations to speak and lecture on Prostatic artery embolization. The schedule will be in October, so I have to sit down next week and start on my powerpoint lecture. My schedule next week will be lighter, so I have time to prepare for the lecture. I will get my notes from the materials you gave me .They are very interested in prostatic artery embolization for BPH and for prostatic bleeders.

Again thank you very much for all the things I have learned from you. My friends who saw your achievements say that with your credentials , I am definitely well trained. Thank you for your hospitality and that of your staff and the Portuguese people that i had opportunity to meet. I hope we will see each other again in the future.

A meeting of PAE on BPH in Escola Superior de Tecnologia da Saúde de Lisboa. The following urologists were present as speakers: Dr. José Dias, Abrantes Monterio, Filipe Rodrigues and Rui Prisco. Eighty four treated patients were present and some invited urologists. Several of the treated patients made good comments of the technique. Every one mentioned significant improvement of the symptoms and of the quality of life.

INTRODUCTION AND OBJECTIVES: Evaluate outcome of prostatic arterial embolization (PAE) for symptomatic patients with benign prostatic hyperplasia (BPH) and very large prostates, after failure of medical therapy for at least 6 months.

METHODS: This prospective nonrandomized study included 72 patients (aged 48 – 80 years, mean age 67.6  6.7 years) with a diagnosis of BPH, very large prostate and moderate to severe lower urinary tract symptoms (LUTS) after failure of medical treatment for at least 6 months, selected for prostatic arterial embolization (PAE) between March 2009 and April 2012. Sixteen patients were in urinary retention and bladder catheter. The procedure was always performed under local anaesthesia and mostly by single femoral approach. Nonspherical Polyvinyl Alcohol Particles (PVA) were used for embolization.

The prostate volume ranged between 100 and 269 cc (mean 129.5 32.3 cc). The International Prostate Symptom Score (IPSS), Quality of Life (QoL), International Index Erectile Function (IIEF), uroflowmetry, (Qmax – peak urinary flow and PVR – post voiding residual volume), Prostatic Specific Antigen (PSA) and prostate volume, were assessed before PAE, at 1, 3, 6 and every 6 months thereafter. Patients were evaluated up to 36 months (mean 12.3 months).

RESULTS: PAE was technically successful in 71 of 72 patients (98.6%). Follow up data was available for 67 patients. Four patients were lost to follow up. From the total 67 controlled patients there were 16 (23.9%) clinical failures, 9 at short term and 7 at medium term. At one month there was clinical improvement in 57/67 patients (85.1%).

Cumulative rates of clinical success at short term were at 3 months 85.5%, at 6 months 81.4%, at 12 months 78.5%, and at medium term at 18 months 69.8% at 24 months 69.8 %, at 30 months 69.8 and at 36 months 69.8 %. There was low morbidity, no major complication and no sexual dysfunction in this group of patients.

CONCLUSIONS: PAE is a minimally invasive procedure with good short and medium term results, for BPH patients with prostates larger than 100cc and moderate to severe lower urinary tract symptoms after failure of medical therapy, and may be considered an alternative to surgery.

The poster on Embolization of BPH was presented at the Congress of European Society of Urology in Milan. Professor Madersbacher,prestigious urologist, considered it a valid and very important work.

For evaluation of any placebo effect it was suggested a study with a slarm arm. There patients will have angiography with embolization. It was suggested a randomized clinical study in patients in acute urinary retention.

3 patients were treated.
3 hours after the treatment the medical team of users could check that all had improved urinary flow with stronger and larger amounts of urine. At this point everyone went home.

The first BPH embolization was performed at Saint Louis Hospital in a 78 years old man, that was in urinary retention with a bladder catheter for 6 months. He had 2 TURP 12 and 10 years before and an open surgery 6 years before he refused surgery. One week after the embolization the bladder catheter was taken out and he could urinate well.

Six patients with BPH were treated by embolization, completing a total of 115 patients already treated with clinical success in 90%. Four of the patients went home late in the afternnon and all of them could urinate much better with good urinary flow. The other 2 patients stayed in the Hospital because they are foreigners. James La-Giglia from Hong Kong. He is 48 years old, a prostate 120 grams, with a tube placed in the bladder trough the abdomen for urine drainage, for 4 months. He did not want to be operated because of the risks of surgery and he stil wants to have children.

The other in Enrique Aguirrezabala, president of the European Economic Counseul who had several urinary retantions with bladder catheter during the last 4 years.

An invitation was received from a Company to open in January at St. Louis Hospital a European Training Centre of Excellence for treatment of BPH.

5 patient were treated. The 1st team of  belgiam medical doctors came for BPH training 2 urologists: Dr. Serge Holz; Dr. Thibaud Saussez and 2 radiologists: Dr. Frank Hammer; Dr. Salvatore Murgo.

Two other doctors were also presente: Dr. Yves Meulebrouck and Dr. Anik van den Genachten.

Three patients were treated, one of them for UK Stanley Stewart.

A group of Medical Directors from the Japonese Company Terumo was present as the last visit of the year. Dr.Hiroshi Tada, Dr.Arthuro Fueyo and Dr. Antonio Vega.

This visit is the follow up of the invitation from Terumo for the Angiography Department of St. Louis Hospital become an Excelence and Investigation for BPH treatment training.

    I heard your presentation at SIR in Chicago and have read your JVIR article. I am an IR physician at USC in Los Angeles and interested in trying embolization for BPH. I have a patient with urinary retention and BPH who refuses surgery and wants me to do it. The only confounding thing is he has a small focus of carcinoma with elevated PSA. I know this was a contraindication for you but would you consider doing it if he refuses all other treatments? Thank you very much. Michael Katz, M.D., Chief IR at USC.

    One of my friends has directed me to you and your website. I have an enlarment of my prostate too and one of my doctors suggested to have surgery, but I don’t thrust the normal procedure. My question now is if you know that are already doctors in Belgium who are following your procedure?
    I will have a new ultrasound scan shortly and I wonder if I may send an electronic copy to you for evalution if no doctor in Belgium is doing any procedure other than surgery.

    I am a 61 year old in good health apart from BPH. My Urologist recommended that I have TURP last year which I declined due to the potential side effects. I’m very interested in the Embolization method for treating BPH. I’m from the UK, as I’m prepared to travel abroad to have treatment I would like to know if it would be possible to have this procedure carried out at Hospital St. Louis?

    Sou Aníbal Marques da Cruz, cunhado de Erivan Leal (brasileiro que o Dr. Tratou em Agosto/Setembro de 2014) e, como sou

    portador de HBP que mais ou menos vinha controlando com 1comp/dia de tansulosina 0,4 mg só que de momento não

    funciona mais a sua ação química nem mesmo aumentando a dose. Já tive vários bloqueios de emissão urinária e passo a

    noite a ir ao WC, pelo que venho por este meio solicitar-lhe uma consulta e posterior intervenção (técnica de bloqueio

    arterial). Encontro-me de momento no Algarve e só irei para Lisboa em Junho. Entretanto já fiz exames analíticos ao sangue

    com PSA=2,80 e amanhã irei fazer eco prostática transrectal, e no caso de serem preciso mais quaisquer exames agradeço

    que me informe. Sei que devo fazer TAC? (pelvico)? Agradeço sua atenção.
